Abstract | ||
Background: Sinus track represents the site ofdrainage of chronic osteomyelitis, and is caused byimproper treatment of acute hematogenousosteomyelitis ended with sequestrum formation,trauma results in compound fracture, post operativecomplications of the bone operation, and / ortuberculous osteomyelitis. Sinus track culture ofchronic osteomyelitis is a very important & difficultsubject to deal with, because collection andcultivation of material need patience and skillful toreach best resultsMthods: Culture comparison of sinus track andoperative specimens was performed on 20 patientswith chronic osteomyelitis. This was conducted toisolate the causative microorganisms of the chronicsinuses discharge.The investigation included a prospective studyduring the period from October 1999- August 2000in Tikrit Teaching Hospital.The aim of the study was to culture comparebetween the reliability of sinus track with bonespecimen obtained by operative method to knowactual causative agent of chronic bone infection.Results: Eighteen patients (90%) had a singleisolated pathogen from their operative and sinusspecimens, while 2 (10%) patients have more thanone organism.Sinus track cultures show the specificity of 100%and predictive value of 91.6%, while sensitivityshows 94.5%, was more dependent in the study.Sinus track cultures were proved to be unreliablesource for the isolation of Staph. epidermidis whileit is proved to be a reliable source for isolation ofother type of bacteria caused chronic osteomyelitis.Isolation of Staph. aureus from sinus track correlatedwith the presence of Staph. aureus in the operativespecimen.Conclusion: Syringe aspiration of material isproved to be more reliable than cotton swab.A cotton swab can miss most of the causativeorganisms especially the anaerobes and specificbacteria as Mycobacterium tuberculosis, or it maycontain some contaminants. Mycobacterial isolationwas limited to only 2 patients in the present study.Keywords:Chronic,osteomyelitis, sinus | ||
